Terms
These Terms apply to the preliminary provider application for Practice Launch by Amber, operated by New York Cosmetology Company LLC.
These Terms govern the application and review process only. They are not the final provider participation agreement, medical director agreement, clinical protocol, patient consent, employment agreement, independent contractor agreement, or final approval to provide services.
Application Review Only
Submitting an application allows Amber to review candidate eligibility, operational fit, credential readiness and onboarding requirements. Submission does not create an acceptance, partnership, employment relationship, contractor relationship, room reservation, client guarantee, revenue guarantee or right to perform services.
Accuracy and Authority
By submitting the form, you confirm that the information provided is accurate to the best of your knowledge and that you are authorized to submit any business, entity, credential, insurance, product or professional information included in the application.
No Patient Information
The application is not a patient-care form. Do not submit patient names, patient contact information, patient medical facts, full client lists, treatment records, conversation screenshots, patient photos, SSNs, passwords, payment card data or other unrelated private information.
Communication Consent
You agree that Amber may contact you about the application by phone, SMS, email or other contact method you provided. SMS and ordinary email should not be used to transmit patient PHI or highly sensitive documents. You may be asked to use a secure upload link, CRM workflow or other approved channel for document transfer.
Documents and Verification
Preliminary photos, screenshots or PDFs may be accepted for initial review. Amber may request original, unredacted or updated documents before final approval, agreement signing, onboarding, credential verification, clinical workflow review or access to any operating environment.
Operating and Clinical Structure
Amber provides a non-medical operating environment and related operational support. Clinical services, scope of practice, medical oversight, protocols, product use and patient-care requirements must follow the approved legal and clinical workflow. Nothing in the application authorizes a provider to perform services outside their license, scope, training, insurance, supervision requirements or approved protocols.
Products, Add-ons and Commercial Terms
Products must satisfy applicable source, storage, expiration, documentation, inventory and approval requirements. Optional services, add-ons, sales support, marketing support, client acquisition, exclusivity, certification support, product ordering, setup fees, subscription fees and post-trial terms are not included unless confirmed separately in writing by invoice, price list, order, appendix, agreement or other written confirmation.
Changes and Final Documents
Amber may update application requirements, review criteria, available services, onboarding steps and operating requirements. Final participation, if approved, will be handled through separate payment, onboarding, credential verification and contract-signing steps.
