Privacy Policy
This Privacy Policy explains how Practice Launch by Amber and New York Cosmetology Company LLC handle information submitted through the preliminary provider application process.
This page applies to provider candidates and business applicants. It is not a patient intake form, patient consent form, or HIPAA Notice of Privacy Practices. Do not submit patient protected health information, patient names, full client lists, SSNs, passwords, payment card data, or unrelated private information through the application form.
Information We Collect
We collect information needed to review provider eligibility, operational fit and onboarding readiness, including:
- contact details and preferred communication channels;
- license type, license status, professional role and procedure scope;
- training, certification, insurance and credential readiness;
- schedule preferences, room-day needs and operating preferences;
- product route, supplier/source and storage documentation readiness;
- selected optional services and notes submitted by the applicant;
- selected file names and credential documents if a secure upload or approved workflow is used.
How We Use Information
Submitted information may be used for candidate review, credential review, onboarding evaluation, operational planning, legal review, follow-up communications, payment-link preparation, CRM setup and coordination of next steps. Submission does not guarantee acceptance, room availability, clients, revenue or approval to perform any service.
Documents, Photos and Redaction
For preliminary review, clear photos, screenshots or PDFs of provider credentials may be acceptable where relevant. You may cover or redact information you do not want to show at this stage. Original or unredacted documents may still be required later for agreement signing, secure onboarding, credential verification, compliance review or final approval.
Sensitive Information and Secure Transfer
The application form is designed to minimize sensitive information. Sensitive documents should be transferred only through an approved secure upload link, CRM workflow or another approved secure process. Ordinary email, SMS and social media messaging should not be used to send patient PHI or highly sensitive documents.
Sharing and Service Providers
Information may be reviewed by authorized Amber team members, operational staff, the supervising or medical-side team where applicable, legal or compliance reviewers, CRM/service providers and other vendors who support application review, onboarding, scheduling, communications or secure document handling. We do not sell provider application information.
Security and Retention
We use reasonable administrative, technical and physical safeguards appropriate for the application workflow. No static website, email, SMS or online workflow can guarantee absolute security. Application information may be retained as needed for review, onboarding, audit, legal, operational or dispute-resolution purposes, unless deletion is required or approved under an applicable process.
Contact and Updates
Use the contact method provided by the Amber team to request updates, correction or deletion review for provider application information. We may update this policy from time to time. The version posted on this page applies to the application workflow at the time it is reviewed.
